Sheffield Visiting Support Service

Sheffield Visiting Support Service is open to all Sheffield residents who have a current or previous history of misuse of substances or alcohol and need housing related support to maintain independent living.

DISC will work closely with other agencies to undertake a comprehensive assessment of need and develop a structured support plan with the client.

95% of Clients, with housing issues, were effectively supported by DISC to avoid homelessness or eviction

The support is designed to be provided for as long as necessary (normally six months) supporting residents through their own personal recovery into independent living. It is flexible, needs led and tailored to the individual.

The service also aims to enable clients to develop the skills required to manage their own accommodation and to live confidently and abstinent in the community.

DISC’s Independent Living Unit provides comprehensive, high quality, housing related support services and when benchmarked exceeds targets set at a national level. Some of our achievements to date are showcased below:

During 2014

95% of Clients, were effectively supported by DISC to improve their physical health, sense of happiness and wellbeing

97% of Clients supported by DISC, better engaged with friends, family and services within their local community as a result

96% of Clients, supported by DISC, were able to maximise their household income through finding work or accessing correct welfare assistance
